Your portion of coverage is paid through pre-tax payroll contributions.
Preventive and Diagnostic Care: Includes two oral exams, two standard cleanings, one set of bitewing X-rays per year and two periodontal maintenances per year following active therapy.
Basic Restorative Care: Includes fillings, root canals, periodontics, extractions and oral surgery.
Major Restorative Care: Includes crowns, bridges, dentures and implants.

The single deductible is the amount payable toward covered expenses for one person before the plan pays benefits for that individual. For the family deductible, once three family members pay a single or per person deductible, no other family member has a deductible in that calendar year.
