You can choose among 3 medical plans:
Your coverage is paid through pre-tax payroll contributions – and, if you choose a HealthSaver plan, you have $0 payroll contributions for employee-only coverage!*
*You will continue to pay part of the coverage cost for your dependents.
Reminder: UnitedHealthcare (UH) will stop processing claims from non-network providers submitted after December 31, 2024.
Affordable Care Act (ACA) Mandated Medications: Drugs under this designation are covered 100% by all Hasbro Medical and Prescription Drug plans. They include certain tobacco-cessation products and contraceptives for women (age limits may apply).
Annual Deductible: Fixed-dollar amount you pay each calendar year before the plan begins paying.
Annual Out-of-Pocket: Maximum amount you and your covered dependents pay each calendar year toward eligible expenses before the plan pays 100% for covered services and prescriptions.
Coinsurance: Portion the plan pays for covered expenses after you pay the annual deductible.
Copay: Fixed amount you pay for services at the time of the service, such as when you’re at the doctor’s office or the pharmacy. Copays apply only to the Hasbro POS Plan.
IRS Regulated Medications: Drugs under this designation bypass deductibles for the HealthSaver plans only and include common prescription drugs, such as Lovastatin, Moexipril and Ticlopidine. The following preventive product categories are not subject to the deductible for the HealthSaver Plans:
Preventive Care Services: Regular checkups and tests to keep you and your family healthy, such as annual physicals; routine vision and hearing exams; routine mammograms or prostate exams; other screenings based on age and gender; flu shots; and other immunizations. These services are not subject to your deductible.
Prior Authorization: Occurs when certain medications require authorization or approval before they are filled based on medical necessity. You may receive a notice of denial until your provider submits acceptable clinical support to Express Scripts. Example drugs include those to treat acne, adult ADHD, narcolepsy or acute pain.
Step Therapy: When you begin drug therapy for a medical condition, you’re required to use the most cost-effective and safest drug therapy and only progress to other more costly therapy if medically necessary. Example drugs include those to treat high cholesterol, high blood pressure and other specialty medications.

If you purchase maintenance medication |
You must obtain your refills through the mail order service or a Retail 90 Network Pharmacy in 90-day quantities. Otherwise, you’ll pay 100% of the cost of your medication, and that cost doesn’t apply to your deductible or out-of-pocket maximum. (This rule doesn’t apply to your initial 30-day prescription for a new medication and up to two 30-day refills.) |

If you purchase a brand-name drug that has a generic equivalent |
The plan requires use of generic drugs when available. You’ll pay the coinsurance for the generic drug, as well as the difference between the cost of the brand-name and generic drug. This difference doesn’t apply to your deductible or out-of-pocket maximum. (This is even if your doctor prescribed the brand-name drug.) |
|
If you purchase certain drugs that have over-the-counter alternatives |
You’ll pay the full cost of your medication, and this cost doesn’t apply to your deductible or out-of-pocket maximum. (This includes drugs such as Aciphex, Nexium or certain formulations of Clarinex and Clarinex D.) |
|
If you purchase certain drugs excluded from the Express Scripts formulary list |
You’ll pay the full cost of your medication, and this cost doesn’t apply to your deductible or out-of-pocket maximum. Find out if a drug is excluded by using the Express Scripts mobile app. |
|
If you need certain specialty drugs available only through mail order |
You’ll pay the full cost of your medication, and this cost doesn’t apply to your deductible or out-of-pocket maximum. Find out if a drug is excluded by using the Express Scripts mobile app. |
For the most up-to-date medication coverage and exclusions, log into your Express Scripts account. You also can go to the Health Plans folder in the Reference Center of your HasbroBenefitsolver account for the preventive medications list, excluded medications and the drug formulary.
TIP: Use your Express Scripts prescription drug card (not your medical card) when getting preventive vaccines at a participating pharmacy.
