If you’re an active employee, you receive Basic Life, Basic Accidental Death & Dismemberment (AD&D) and Business Travel Accident Insurance automatically, you don’t need to enroll.

You can purchase Supplemental Life, Supplemental AD&D and Dependent Life Insurance for yourself and your family.

Evidence of Insurability (EOI): The process by which Aflac determines if you are healthy enough to be considered eligible for the amount of insurance coverage you are requesting. 

Imputed Income: The taxable amount of your Basic, Supplemental and Dependent Life Insurance premium value over the Internal Revenue Service limits. If this applies, the amount will appear on your pay stub and W-2 form.

You can cover yourself, your spouse/domestic partner and your dependents through the end of the month in which they turn age 26.

  • You must be actively working for coverage to begin or for any increase in existing coverage to become effective. 
  • Both your Basic Life and AD&D Insurance coverages are equal to one times your base salary, rounded to the next higher $1,000, up to $1,000,000. 
  • Coverage is updated annually on January 1 using your base salary as of September 1 to reflect changes in your compensation.  
  • Current tax laws require that you pay federal income and Social Security (FICA) taxes on the premium value of your Basic Life Insurance benefit over $50,000. 
  • In the event of your accidental death, the entire amount is paid to your beneficiary. For other losses resulting form an accident, the amount paid depends on the type of loss, such as sight, limb, speech or hearing. AD&D Insurance is not subject to imputed income. 
  • EOI is not required.
  • Provided by AIG at no cost to you. 
  • Protects you when you are traveling on company business.
  • Coverage equals five times your annualized base salary with a minimum benefit of $100,000 and a maximum benefit of up to $500,000.*
  • Does not cover your daily commute, and other exclusions apply

*Benefit amount depends on the number of insured participants involved in a singular accident.

  • You must be actively working for coverage to begin or for any increase in existing coverage to become effective, with EOI. 
  • You can purchase coverage for yourself, one to eight times your base salary, up to $2,000,000. 
  • During Annual Open Enrollment , held in the fall, you can elect or increase your Supplemental Life by one multiple of your base salary, up to the lesser of five times your base salary or $750,000.
  • For marriage or birth/adoption, you may enroll in or increase your coverage by one multiple of base salary unless your current coverage is five times base salary or $750,000. An increase over five times base salary or $750,000 always requires EOI and approval by Aflac. 
  • Cost is based on your age (as of December 31), and contributions are paid through after-tax payroll deductions. 
  • You can enroll in Supplemental coverage at any time, with EOI. 
  • The beneficiary you designate for Basic Life and AD&D Insurance is also the beneficiary for your Supplemental Life Insurance. 
  • Supplemental Life Insurance may be subject to imputed income.

NOTE: If your spouse/domestic partner works for Hasbro, he/she must enroll as an employee in the Supplemental Life and Supplemental AD&D plans.

For your spouse/domestic partner:

  • Purchase in increments of $10,000 up to $100,000. 
  • Premium rates for the calendar year are based on your spouse’s/domestic partner’s age as of December 31. 
  • Within your first 31 days of eligibility, you may enroll in up to $50,000 coverage without EOI. 
  • During Annual Open Enrollment, held in the fall, you can enroll in or increase your spouse/domestic partner coverage by one increment of $10,000 up to $50,000 without EOI. 
  • After your first 31 days of eligibility and outside of Annual Open Enrollment, you may enroll in or increase your coverage at any time. EOI and approval by Aflac will be required regardless of the coverage amount. 
  • For marriage or birth, you may enroll in up to $50,000 without EOI. 
  • Your increased coverage will become effective on the date Aflac, approves your application. 
  • You are automatically the beneficiary for any covered family members. 
  • Spouse/domestic partner Dependent Life insurance may be subject to imputed income.

For your children/domestic partner’s children:

  • Purchase in increments of $2,500 up to $25,000 at any time without EOI. 
  • One election and one premium covers all of your eligible children from age 14 days through the end of the month in which they turn age 26. 
  • You are automatically the beneficiary for any covered family members.

You must be actively at work for coverage to begin or for any increase in existing coverage to become effective.

For yourself:

  • Purchase one to eight times your base salary up to $2,000,000, and EOI is not required. 
  • Enroll any time. 
  • Coverage is not subject to imputed income. 

For your spouse/domestic partner: Coverage is 50% of your Supplemental AD&D coverage amount.

For your children: Coverage is 15% of your Supplemental AD&D coverage amount up to a maximum benefit of $25,000 per child.

For your entire family:

  • Your spouse’s/domestic partner’s coverage1 is 40% of your Supplemental AD&D coverage amount. 
  • Each child’s coverage is 10% of your Supplemental AD&D coverage amount up to a maximum benefit of $25,000 per child. 

1In the event of death, the entire amount is paid to you as the beneficiary. In the event of dismemberment, the amount paid to you depends on the type of loss.

WHEN EOI IS REQUIRED FOR SUPPLEMENTAL LIFE INSURANCE COVERAGE

Within your first 31 days of eligibility

You may enroll in coverage up to the lesser of five times base salary or $750,000 of Supplemental Life Insurance without EOI. For coverage amounts above $750,000, EOI and approval by Aflac are required. 

During Annual Open Enrollment

You can elect or increase supplemental coverage by one multiple of your base salary up to the lesser of five times your base salary or $750,000. EOI and approval by Aflac are required for any amounts greater than five times base salary or $750,000.

Qualifying Life Event

You may enroll in or increase your coverage by one multiple of base salary unless your current coverage is five times base salary or $750,000. This applies for marriage or birth/adoption only. An increase over five times base salary or $750,000 always requires EOI and approval by Aflac. 

Any Time

You may enroll in or increase coverage at any time. EOI and approval by Aflac will be required regardless of the amount of coverage.

George and his wife Diane just adopted a son. He wants to increase his wife’s insurance coverage and add his son to his coverage. 

What steps should George take to make the most of his insurance benefits?

DON’T FORGET: It’s important to designate a beneficiary for your Life Insurance benefits.

However, be cautious when naming a minor child as your beneficiary. Generally, federal and state laws prevent insurance companies from paying benefits to minors until age 18. Consider naming a custodian for a minor child ad/or create a trust with a trustee to disburse your assets according to your wishes.
